2. Fatty Diet

A diet high in fried food, fast food, processed snacks, and fatty cuts of meat causes your body to store extra fat, some of which collects in your liver. Foods high in saturated fat, including butter, full‑fat dairy products, and fatty red meat, raise fat levels in your liver. Trans fats found in baked goods and packaged foods increase the risk even more.
Eating too much sugar also causes your body to turn the excess into fat that can build up in your liver over time. Sugary drinks, candy, and refined grains add to this fat storage process.
Fatty liver disease often develops slowly and without pain. If you have obesity, high blood pressure, or type 2 diabetes, a fatty diet raises your risk even more. Extra body fat, especially around your belly, is strongly linked to fat buildup in the liver. Over time, this can lead to liver inflammation, scarring, and lasting liver damage that affects overall body health.
3. Autoimmune Disease
Autoimmune disease can harm your liver when your immune system attacks your own liver cells, which leads to swelling and damage inside the liver. In autoimmune hepatitis, long-term inflammation can cause scarring (cirrhosis) and increase your risk of liver failure if left untreated. Chronic inflammation changes how your liver handles fat, making it harder to process and move fat normally, which may lead to fatty liver disease.
You may notice symptoms such as tiredness, yellowing of the skin or eyes, dark urine, or joint pain, though some people have no clear symptoms at first. Blood tests often show high liver enzymes before you feel sick.
Doctors often treat autoimmune liver disease with steroid medicines and immune-suppressing drugs to control the immune attack and limit further fat buildup and scarring. If you already have another autoimmune condition, your risk may be higher.
4. Drugs

Some medicines can cause fat buildup in your liver. Your liver breaks down most medicines, so it can get injured if drug byproducts build up. High doses of pain relievers, especially when you exceed recommended amounts, can harm your liver, and the risk rises if you drink alcohol often. Nonsteroidal anti-inflammatory drugs have also been linked to liver injury in some people.
Other drug classes that may affect your liver include certain antibiotics and some anti-seizure medicines. Cholesterol-lowering agents, hormonal birth control medicines, anabolic steroids, some cancer treatments, and drugs used to treat heart rhythm problems can also place stress on the liver.
These medicines do not always cause fatty liver, but they can trigger liver stress or inflammation, which may lead to fat buildup or more serious liver damage in some people. Blood tests often show high liver enzyme levels before you feel sick. If a drug is the cause, your liver often improves after you stop taking it under medical guidance.
5. Genetics
Your genes can raise your risk of fatty liver disease, even if you do not drink alcohol. If close family members have fatty liver, cirrhosis, or type 2 diabetes, your risk may be higher, showing that inherited traits affect how your body stores and uses fat.
Certain gene changes, like those in the PNPLA3 gene, can lead to more fat buildup in liver cells, while other variants affect how your body breaks down fats or responds to insulin. You may inherit genes that make you more likely to gain weight, develop insulin resistance, or have high blood fat levels, which increases the chance that fat will collect in your liver.
Genes do not act alone; your diet, weight, and activity level still play a large role. Fatty liver does not follow a simple inheritance pattern, but many small genetic changes combine with lifestyle factors to shape your risk.
6. Sodium

Your body needs a small amount of sodium to control fluid balance and nerve signals, but too much can harm your liver over time. Most people get far more sodium than they need each day, often from processed and restaurant foods like canned soups, deli meats, frozen meals, chips, and fast food. Even foods that do not taste salty, like bread and sauces, can contain large amounts.
When you eat too much sodium, your body holds on to extra water, which raises your blood pressure and strains your blood vessels. Over time, high blood pressure can reduce healthy blood flow to your liver and worsen damage. Diets high in sodium often go along with diets high in calories and unhealthy fats, which increases your risk of fat buildup in the liver. If you already have fatty liver disease, too much sodium can make fluid retention worse, especially in later stages like cirrhosis, and this leads to swelling in your legs or abdomen.
7. High Cholesterol
High cholesterol means you have too much fat-like substance in your blood, and when levels stay high, extra fat can build up in your liver. Nonalcoholic fatty liver disease often links with high cholesterol, and many people have no symptoms.
Cholesterol travels through your blood in different forms. LDL is known as “bad” cholesterol because it can build up in blood vessels. HDL is called “good” cholesterol because it helps remove extra cholesterol from the bloodstream. Triglycerides are another type of fat that often rises with a poor diet and weight gain.
High LDL and high triglycerides raise your risk for both heart disease and fatty liver, especially if you also have obesity, type 2 diabetes, or metabolic syndrome. Cholesterol levels can rise due to a diet high in saturated fat and processed foods, low physical activity, excess body weight, and a family history of high cholesterol.
8. Toxins

Toxins can harm your liver and lead to fat buildup. Your liver breaks down and removes harmful substances from your body, but high or repeated exposure can stress your liver and cause excess fat storage. Alcohol is a common toxin linked to fatty liver, and heavy or long-term drinking raises your risk of alcohol-related fatty liver disease. Certain chemicals, such as industrial solvents, pesticides, and some cleaning agents, also raise risk. Breathing in fumes or touching these chemicals without protection strains your liver over time.
Some drugs, like corticosteroids, chemotherapy agents, and certain antibiotics, may increase fat buildup in the liver, especially if you already have other risk factors like obesity or diabetes. Natural toxins, such as mold toxins in spoiled grains or nuts, may damage liver cells if you consume them in large amounts. Your risk depends on how much toxin you face and how long you are exposed. Repeated exposure makes it harder for your liver to recover fully.
9. Obesity

Obesity is one of the most common causes of fatty liver disease. When you carry excess body fat, your body stores more fat than it can properly use, and some of this fat collects in your liver. Your liver processes fats and sugars from the food you eat, but obesity can make your body less sensitive to insulin. This raises fat levels in your blood and increases fat buildup in your liver.
Fatty liver linked to obesity is often called nonalcoholic fatty liver disease. Many people have no symptoms at first. Risk rises when your body mass index falls in the obese range or when you carry extra weight around your waist. Type 2 diabetes, high blood pressure, and high cholesterol or high triglycerides also increase the likelihood of fat buildup in the liver.
Children and teens with obesity can also develop fatty liver, and the risk rises as body weight increases. Not everyone with obesity develops fatty liver, but the chance goes up as your weight increases, especially if you also have other metabolic problems.
10. Insulin Resistance
Insulin resistance occurs when your body stops responding well to insulin, the hormone that moves sugar from your blood into your cells for energy. As this process slows, sugar builds up in your blood and your body produces more insulin to compensate. High insulin levels can signal your liver to store more fat, which may lead to fatty liver disease over time.
Insulin resistance often appears before type 2 diabetes and is linked to prediabetes. Many people with fatty liver disease also have high blood sugar, high cholesterol, or high blood pressure.
Common risk factors include excess body weight, low physical activity, a diet high in added sugars and refined carbohydrates, and a family history of type 2 diabetes. As liver fat increases, your body may have even more trouble controlling blood sugar levels.
Doctors may treat advanced fatty liver linked to insulin resistance with a thyroid hormone receptor-beta agonist, along with diet and exercise. Managing blood sugar and improving insulin response play a key role in slowing liver damage.
11. High Blood Glucose Levels
High blood glucose levels can raise your risk of fatty liver disease. When you have too much sugar in your blood for a long time, your body struggles to manage it, often due to insulin resistance, prediabetes, or type 2 diabetes. Your liver plays a key role in blood sugar control by storing extra sugar after you eat and releasing it later to keep your energy steady.
When blood glucose stays high, this system becomes unbalanced. Over time, excess sugar turns into fat, which your liver stores; if too much fat builds up, fatty liver disease can develop. Many people with type 2 diabetes also have fatty liver, and the two conditions often occur together. Fat buildup in your liver can make blood sugar control harder. You may not notice symptoms in the early stages, as fatty liver disease often develops slowly and without clear signs, even while blood sugar levels remain high.
12. Polycystic Ovary Syndrome

Polycystic ovary syndrome (PCOS) is a common hormone condition that affects how your ovaries work, often starting in your teen years or early adulthood. Many women with PCOS have irregular periods and do not ovulate regularly. PCOS is closely linked to insulin resistance, which can lead to several health complications.
When your body does not respond well to insulin, it produces more of it. High insulin levels raise male-type hormones, which may cause acne, extra facial or body hair, and weight gain. This insulin problem also increases your risk for nonalcoholic fatty liver disease. Extra insulin and weight gain cause fat to build up in your liver, and over time, this fat can lead to liver swelling and damage if not managed. PCOS affects about 1 in 10 women of childbearing age. Many women with PCOS are overweight, but not all.
