2. Urinary Tract Infection

A urinary tract infection occurs when bacteria enter the urinary system and begin to grow. The urinary tract includes the kidneys, ureters, bladder, and urethra. Most infections start in the bladder, but bacteria can move upward and reach the kidneys if the infection spreads.

Klebsiella pneumoniae can cause urinary tract infections, especially in hospital or healthcare settings. This bacterium normally lives in the intestines without causing harm. Infection develops when it enters the urinary tract, where it can attach to the lining of the bladder or kidneys and multiply.

In healthcare environments, Klebsiella pneumoniae can survive on surfaces or medical equipment. People who use urinary catheters face a higher risk because the tube provides a direct path for bacteria to reach the bladder. The longer a catheter remains in place, the higher the risk of infection.

Common symptoms include pain or burning during urination, frequent urges to urinate, pressure in the lower abdomen, and cloudy or foul-smelling urine. Some people may also notice blood in the urine. If bacteria spread to the kidneys, symptoms may worsen and include fever, back or side pain, nausea, or chills.

3. Soft Tissue or Skin Infection

Klebsiella pneumoniae can infect the skin and the soft tissue beneath it when the skin’s protective barrier is broken. This often occurs after surgery, injury, or other breaks in the skin, allowing bacteria to enter and multiply in the surrounding tissue. Moist or poorly healing wounds provide an environment where bacteria can grow more easily.

People in hospitals face a higher risk of these infections. Long hospital stays, weakened immune systems, recent surgery, and the use of medical devices such as intravenous lines or drains increase exposure to the bacteria. Some infections remain close to the surface and cause localized redness, swelling, warmth, or pain.

The skin may also appear shiny or tight, and fluid or pus may drain from the area. If not treated, the infection can spread deeper into muscle or enter the bloodstream, raising the risk of serious complications such as cellulitis or sepsis.

4. Meningitis

Meningitis is an infection and inflammation of the thin protective lining that surrounds the brain and spinal cord. Klebsiella pneumoniae can cause this condition, although it is less common than other bacterial causes.

When it does occur, it is usually severe and considered a medical emergency. It most often affects people who are already very ill, hospitalized, or receiving intensive medical care. The infection usually develops after the bacteria spread through the bloodstream and reach the membranes of the brain and spinal cord.

This process is known as bloodstream invasion and allows bacteria to cross protective barriers around the brain. Meningitis can also occur after brain surgery, head injury, or the use of medical devices such as drainage tubes or shunts placed in the brain.

Once the bacteria reach the meninges, inflammation can develop quickly. This swelling may raise pressure inside the skull and affect normal brain function. Symptoms may include high fever, severe headache, neck stiffness, confusion, sensitivity to light, or vomiting. Because the condition can worsen rapidly, immediate medical treatment is needed.

5. Endophthalmitis

Endophthalmitis is a serious infection inside the eye. Klebsiella pneumoniae can cause this condition when it enters the eye directly after surgery, medical procedures, or injury, or when it spreads through the bloodstream from an infection elsewhere in the body. When the bacteria spread through the blood, the condition is called endogenous endophthalmitis. 

This form is more common in people with diabetes, liver disease, or weakened immune systems. Hospital-related factors, including invasive eye procedures and poor infection control, can further increase the risk. The bacteria spread through direct contact rather than through the air. Symptoms often develop rapidly and may worsen within hours.

These include eye pain, redness, swelling, sensitivity to light, and blurred vision. Vision loss can occur quickly if treatment is delayed. As the infection progresses, inflammation can involve the inner eye structures, including the vitreous and retina. This can cause increasing pressure inside the eye and further damage to delicate tissues responsible for vision.

6. Pyogenic Liver Abscess

A pyogenic liver abscess is a pocket of pus that forms inside the liver due to a deep bacterial infection. Klebsiella pneumoniae is a common cause of this condition, particularly in people with certain risk factors. The bacteria reach the liver through the bloodstream or bile ducts and begin to multiply within liver tissue, leading to localized inflammation.

Many cases originate from infections elsewhere in the abdomen. Conditions such as appendicitis, diverticulitis, gallbladder infection, or a blocked bile duct can allow bacteria to spread to the liver. In some cases, no clear source is identified, and the bacteria reach the liver through the bloodstream from another infected site.

People with diabetes or weakened immune systems are at higher risk. Recent hospital stays, bile duct procedures, liver disease, or long courses of antibiotics can further increase susceptibility. Symptoms usually develop gradually over several days and may include fever, chills, pain in the upper right abdomen, nausea, fatigue, and jaundice.

7. Blood Infection

Klebsiella pneumoniae can enter the bloodstream and cause a serious condition called bacteremia. This most often begins when the bacteria spreads from another infected site, such as the lungs, urinary tract, or a surgical wound. Once in the bloodstream, the bacteria circulates throughout the body and can reach vital organs, including the heart and brain.

Because the infection can worsen quickly, urgent medical care is needed. Hospital-related factors increase risk. Intravenous catheters, breathing machines, and other medical devices can allow bacteria to enter the bloodstream. Long courses of certain antibiotics can further raise risk by disrupting normal bacteria that help protect against infection.

As the bacteria multiply in the blood, the body may develop a widespread inflammatory response. Symptoms can include fever, chills, rapid heartbeat, low blood pressure, confusion, or extreme weakness. If the infection spreads further, it can lead to sepsis, a life-threatening condition that affects multiple organs and requires immediate treatment.

8. Person-to-Person Contact

Person-to-person contact is one of the most common ways Klebsiella pneumoniae spreads, particularly in hospitals and long-term care facilities where close interaction is common. The bacteria often passes from one individual to another through direct contact. Many people carry Klebsiella in their stool without feeling sick.

When proper hand hygiene is not followed, the bacteria can transfer from contaminated hands to another person or nearby surfaces, then enter the body through the mouth, nose, eyes, urinary tract, or open wounds. Although Klebsiella does not spread through the air, it can survive on hands and frequently touched surfaces.

Patients who share rooms, bathrooms, or medical equipment therefore face a higher risk of exposure. Healthcare workers can transfer the bacteria between patients if hands are not cleaned properly. Gloves help reduce risk, but proper handwashing or use of hand sanitizer before and after patient contact is essential to prevent spread.
