2. Causes of Baker’s Cyst

Extra joint fluid in your knee can build up and push into the space behind your knee, which creates a swelling called a Baker’s cyst. This fluid, called synovial fluid, normally helps your knee move smoothly, but when your knee makes too much of it, a cyst can form.
Common causes include:
- Osteoarthritis, which happens from wear and tear over time
- Rheumatoid arthritis, an inflammatory joint disease
- Knee injuries, such as a torn cartilage or ligament
- Repeated stress from sports or heavy use
If you damage your knee during sports or from a fall, your body may respond by producing more fluid. This increases pressure inside the joint and forces fluid into the back of your knee. In children, Baker’s cysts often develop without a clear joint disease, and minor trauma or irritation around the knee may play a role. In adults, the cyst usually links to joint damage or long-term joint conditions.
3. Diagnosis
Your doctor often finds a Baker’s cyst during a physical exam. You stand and lie down while the doctor checks the back of your knee for a smooth, round swelling in the hollow behind your knee. The lump may feel soft and filled with fluid, and it shows more when you stand and feels tighter when you fully straighten your leg. When you bend your knee, the swelling can become less firm or less visible.
Your doctor will press on the area to check for tenderness and compare both knees for signs of joint damage, such as stiffness, warmth, or reduced movement. If the cause is not clear, imaging tests can help. An ultrasound can confirm that the lump contains fluid and not solid tissue, and it can help rule out a blood clot in your leg.
An MRI may be used if you have ongoing knee pain or signs of cartilage or ligament injury, as this scan shows detailed images of the joint and helps identify problems like arthritis or a meniscus tear.
4. Treatment

Your treatment depends on your symptoms and the cause of the cyst. Some popliteal cysts do not need medical treatment and may shrink on their own, especially in children. If you have mild pain or swelling, simple home care can help. Resting your knee, avoiding activities that worsen pain, applying an ice pack wrapped in a cloth for about 10 minutes at a time, and raising your leg on a pillow can reduce swelling. Over-the-counter pain relievers such as nonsteroidal anti-inflammatory drugs or acetaminophen can ease discomfort.
If an underlying knee problem caused the cyst, your doctor will treat that condition, possibly with physical therapy to improve strength and movement. In some cases, your doctor may give a corticosteroid injection into the knee joint to lower swelling. When symptoms continue or limit your movement, your doctor may drain fluid from the cyst with a needle. Surgery is rare, but it may be needed if you have ongoing pain or joint damage.
5. Home Remedies
You can ease symptoms of a popliteal cyst at home, especially if your pain is mild. Simple steps often reduce swelling and help you move your knee with less discomfort, starting with rest. Limiting activities that strain your knee, such as long walks, squatting, or heavy lifting, can reduce pressure inside the joint. Giving your knee time to recover may help lower fluid buildup behind it.
Cold packs can help control swelling. Placing an ice pack or a bag of frozen vegetables wrapped in a thin towel on the back of your knee for 10 to 15 minutes at a time may reduce inflammation. This can be repeated every few hours if needed.
Keeping your leg elevated while sitting or lying down may also reduce swelling. Positioning your knee above heart level with the support of a pillow can improve fluid drainage. Over-the-counter pain relievers, such as nonsteroidal anti-inflammatory drugs (NSAIDs) or acetaminophen, may ease pain and stiffness.
6. Recovery Period
Your recovery time depends on the size of the cyst and the cause behind it. Some cysts shrink on their own within a few weeks, while others may take several months, especially if arthritis or a knee injury continues to irritate the joint. If your cyst does not need surgery, you can expect gradual improvement. Resting your knee, using ice for short periods, and keeping your leg raised can ease swelling. Over-the-counter pain relievers can help control discomfort.
If a doctor drains the cyst or gives a corticosteroid injection, you may feel relief within days, but the cyst can return if joint swelling continues. Physical therapy can support recovery by improving knee strength and movement. If you need surgery, healing may take several weeks. You may need to limit activity and follow a guided exercise plan. Most people return to normal daily tasks once pain and swelling improve.
7. Complications
Most popliteal cysts do not cause serious problems, but complications can occur if the cyst grows, leaks, or presses on nearby tissue. One common issue is ongoing pain or stiffness behind your knee. The area may feel tight, especially when you bend or straighten your leg, which can limit how far you move your knee and affect daily activity.
The cyst can also rupture. When this happens, fluid leaks into your calf, and you may notice sudden sharp pain, swelling, redness, or bruising in the back of your leg. These symptoms can look like a blood clot in a deep vein, which is a serious condition that also causes calf pain and swelling.
In some cases, the cyst presses on nearby structures. This pressure can cause swelling in your lower leg due to reduced blood flow. It may also lead to numbness or tingling if a nerve is compressed. Increased pressure in the calf muscles can cause severe pain. Bleeding into the cyst may occur, especially if you take blood-thinning medication. Infection is rare but possible, sometimes after a steroid injection into the knee.
8. Risk Factors

You face a higher risk of a popliteal cyst if you have a knee problem that causes extra joint fluid. Extra fluid can collect in the back of the joint and form a swelling. Certain knee conditions and lifestyle factors increase this risk, including osteoarthritis, rheumatoid arthritis, gout, meniscus tears, and other knee injuries. If you have arthritis, your joint lining can become irritated, which increases fluid inside your knee. Over time, that pressure pushes fluid into the space behind your knee.
Age also plays a role. Adults between 35 and 70 develop these cysts more often, especially if they already have joint wear and tear. Children can get them too, but this is less common and often not linked to joint damage.
Repeated knee strain from work, sports, or frequent kneeling can also raise your risk. Ongoing stress leads to small injuries that increase joint fluid. Inflammatory conditions that affect joints throughout your body can raise your risk as well.
